Payment and Appointment Policies
Payment Terms
Full payment is required prior to each scheduled appointment.
Refund Policy
Prepaid sessions are refundable on a prorated basis within 30 business days, provided a written request is submitted.
Late Cancellations
Appointments canceled less than 48 hours prior will not be refunded, but may be rescheduled.

Patients who are more than 10 minutes late for any appointment time are required to reschedule the visit and may be charged a $25 rescheduling fee.
Missed Appointments
Failure to attend or notify the provider at least 24 business hours in advance of an appointment will result in forfeiture of payment. Missed appointments are non-refundable and cannot be rescheduled.
Insurance Copay
Insurance Copays are not refundable, and patients may be charged a $50 missed appointment fee for failure to attend or notify the provider at least 24 business hours in advance of an appointment.
Privacy & Liability
Confidentiality and Intellectual Property
Phoenix Metabolic Health PLLC will protect Patient information in compliance with HIPAA regulations.
Patient may not record, reproduce, distribute, or sell any materials, digital content, or resources provided by Phoenix Metabolic Health PLLC.
Limitations of Liability
Phoenix Metabolic Health PLLC provides telemedicine services as part of a broader healthcare approach. The Provider is not liable for outcomes resulting from incomplete or inaccurate information provided by the Patient.
The Patient understands that telemedicine has limitations compared to in-person care. Phoenix Metabolic Health PLLC does not provide emergency services or manage life-threatening conditions. In the event of an emergency, patients should contact 911 or visit the nearest emergency facility.
Governing Law and Dispute Resolution
Governing Law and Dispute Resolution: This Agreement shall be governed by and construed in accordance with the laws of the State of Arizona. Any disputes arising under or related to this Agreement shall be resolved through binding arbitration in Maricopa County, Arizona.
